hereinafter referred to as “child”, hereby give my/our child permission and authorization to South Laurel High School’s Director of Bands and/or any of the designated chaperones to transport my child on any band functions from July 1st, 2020 through June 30th, 2021. I/We release and discharge the Director of Bands and/or any of the designated staff or chaperones from any and all claims, demands, actions, or causes of action, arising form any trip.
